New Patients

If you wish to register at the practice and are within the practice boundary, Please print off a copy of our Patient Registration form GMS1 plus New Patient Registration Form. You will be asked to complete the GMS1 form AND the New Patient questionnaire. You will need to bring along photographic ID and confirmation of your address to register.

PRACTICE STATEMENT– Named GP

All patients at Greasby Group Practice have a named GP which will be one of the five partners. You can ring and ask who your named GP is and a Receptionist will be happy to advise you of this.If you would prefer a different GP to the one you have been allocated, please contact the practice.  We will endeavor to accommodate your request, where we can. By having a named GP does not prevent you from seeing any other GP at the practice.

PATIENTS MOVING OUTSIDE OUR CATCHMENT AREA

It is the practice policy that if patients that are registered at the practice move from living within the practice area to outside the practice area they will be de-registered from our practice list on the grounds that the patient has moved out of our area. It is the patient’s responsibility to re-register at a new GP surgery in the area where they are now living in order to receive continuing care.

The NHS PCSE will be able to provide you with a list of GP’s in your area to help you find a new GP. They can be contacted on 0333 0142884. Alternatively if you have access to the internet you can find GP information on the NHS Choices website http://www.nhs.uk/Service-Search/GP/LocationSearch/4

Patient preference of practitioner – you can request a doctor of your choice but this will depend on availability.

Patients not seen within 3 years may request a health check with a Practice Nurse.

Patients aged 75 years and older may request a health check every year with the Practice Nurse if they are not already under regular review. A home visit may be required at the doctor’s discretion. Patients of 75 and over will have a named GP who will have overall responsibility for their care. Please ask if you are unsure who your named GP is.

You may print off a GMS1, fill it out and bring it in with you on your first visit to the practice.
