Before you can start using online facilities, you must register your details for this service.
It’s quick and easy to register, simply follow the steps below:
Ask at reception for a registration form or alternatively download the registration form. This form provides all the information you need to register to use the Patient Access internet facilities. You will need to return the form to reception, bringing with you two forms of ID: one photo and one utility bill or other form of correspondence with your name and address on.
Once you have completed the form and returned it back to the practice they will issue you with the following information:
PIN
Practice ID Number
Access ID
NHS Number
Please go to the Patient Access website to register your account and enter your personal registration details. If the information does not match, your registration will not be accepted.

Proxy access to your own record
You may find it helpful if someone else, usually a family member, close friend or carer, has online access to your GP record so that they can book appointments for you, order repeat prescriptions, or even to access your detailed care record on your behalf to help with your care. This is called proxy access.
The practice can consider proxy access to the GP record we hold here for any patient who is, in the opinion of their usual GP, freely able to give their fully-informed consent for their nominated proxy. If you are the patient and you wish to do this, you should complete the form (Please insert the form here)and return it to your usual GP to formally request this. Your GP may wish to discuss the matter with you before consenting.
