Privacy Leaflet for Children and Young People
The General Data Protection Regulation 2018 (GDPR) gives every person the right to apply for access to their own health records. A request for access can be made verbally, in writing or electronically to your GP Practice for GP records.
Every person applying for access to a health record will be asked for evidence of their identity, this must be two items of ID, photo ID and confirmation of address.
